If you plan to hold a function during the RSNA 2008 Annual Meeting, you need to complete an Affiliate Function Space Request Form. Forms must be received by Friday, November 14, 2008.
AGAIN FOR THIS YEAR…The Affiliate Function Space Request Form can be submitted by e-mail. Listed below are some helpful steps:
- Complete the Function Space Request Form by typing your meeting space requirements.
- Scroll over the "Submit by E-mail" button to read important information and then click on the button.
- Select an e-mail client. The most frequently used application will be the "Desktop E-mail Application".
- Click the "Send Data File" button. (If you have filled out *.PDF forms in the past, you may have "turned off" this box and it will not appear). You can also print the form here.
- Your e-mail program should open with an e-mail pre-addressed to meetingservices@rsna.orgwith an attached data file. (You may have a "Compose E-mail" tab that appears on your taskbar, generally at the bottom of the screen, which you need to click on).
- Send the e-mail as you normally would, along with the attachment.
- Once you have sent the e-mail with the attached data file, do not make changes electronically and resend. If you need to make changes to your function, please write the changes on the original form you printed and fax to the RSNA meetings department at (630) 571-7837.
- Remember to print a copy of the form for your records.
- Confirmation of space at the McCormick Place Convention Center will be mailed in late September. Confirmation of space at a hotel may take up to 3 weeks from the date received.
